Pre hearing information

Allegation

The tribunal will inquire into the allegation that on 26 February 2019 Dr Demanya failed to provide appropriate care and treatment to Patient A at Royal Glamorgan Hospital. It is further alleged that Dr Demanya dishonestly made retrospective entries to Patient A’s prescription chart and medical record.

It is further alleged that on 25 February 2020 Dr Demanya acted dishonestly in that he made a false representation to the Coroner regarding his care of Patient A.

Bydd y tribiwnlys yn ymchwilio i’r honiad bod Dr Demanya, ar 26 Chwefror 2019, wedi methu â darparu gofal a thriniaeth briodol i Glaf A yn Ysbyty Brenhinol Morgannwg. Honnir ymhellach bod Dr Demanya wedi gwneud cofnodion ôl-weithredol i siart presgripsiwn a chofnod meddygol Claf A yn anonest.

Honnir ymhellach bod Dr Demanya wedi ymddwyn yn anonest ar 25 Chwefror 2020 yn yr ystyr iddo wneud sylw ffug i’r Crwner ynghylch ei ofal o Glaf A.
