Pre hearing information

Allegation

The tribunal will inquire into the allegation that, between December 2015 and February 2018, Dr Kocialkowski failed to provide good clinical care to thirteen patients. It is alleged that Dr Kocialkowski’s failings related variously to informed consent, record keeping, history taking, treatment and examination.

It is further alleged that Dr Kocialkowski undertook a procedure and prescribed medication for a patient with whom he had a close personal relationship.
