Pre hearing information
The tribunal will inquire into the allegation that, on 20 May 2017, Dr Ojo engaged in a telephone consultation with Patient A’s mother in relation to Patient A. It is alleged that Dr Ojo failed to obtain an adequate medical history or adequately assess or diagnose Patient A. Further, it is alleged that Dr Ojo failed to follow relevant guidance or make an adequate record of the consultation.
It is also alleged that Dr Ojo prepared a witness statement, dated 12 June 2017, in which he made false statements. It is further alleged that, on 19 July 2018, at the coroner’s inquest, Dr Ojo maintained that the statements he made in his initial statement were accurate. It is alleged that Dr Ojo’s actions were dishonest.
This reflects the allegation as it stands at the start of the hearing. The allegation may be amended as the hearing proceeds and when findings of fact are made by the tribunal.
