Pre hearing information
The tribunal will inquire into the allegation that, on 3 February 2017, Dr Rajanna made a decision to discharge Patient A which was inappropriate as Patient A’s observations raised concerns as to the safety of discharge. It is alleged that on 4 December 2017, Dr Rajanna failed to provide good clinical care to Patient A.
It is alleged that his witness statement to the Pennine Acute Hospitals NHS Trust, dated 3 November 2017, Dr Rajanna made statements which were untrue. It is further alleged that, during his oral evidence at the Coroner’s inquest into Patient A’s death, on 6 March 2018, Dr Rajanna made statements which were untrue. It is alleged that his actions were dishonest.
This reflects the allegation as it stands at the start of the hearing. The allegation may be amended as the hearing proceeds and when findings of fact are made by the tribunal.
