The tribunal will inquire into the allegation that on several separate occasions between the dates of 3 November 2014 and 17 November 2016, Dr Mojahid failed to provide good clinical care to 10 patients by either failing to properly assess and consider their conditions when prescribing medication and prescribing medications without a clear clinical rationale. It is also alleged Dr Mojahid did not record adequate information in a number of the patients medical records.
It is further alleged that Dr Mojahid prescribed medication knowing it would be used by a different patient.
