Pre hearing information
The tribunal will inquire into the allegation that on 6 June 2018, Dr Banks saw Patient A on three or more occasions and he failed to obtain an adequate and appropriate medical history, examine Patient A, take into consideration the GP referral letter, review blood test results and make an appropriate diagnosis for Patient A. It is also alleged that Dr Banks failed to make the necessary referral to the Crisis Mental Health team, devise an adequate and appropriate treatment and management plan and keep an adequate medical record. It is further alleged that Dr Banks’ behaviour towards Patient A was inappropriate. It is also alleged that on 17 June 2018 Dr Banks threatened Patient B.
It is alleged that on 17 December 2018, at Caernarfon Magistrates Court, Dr Banks was convicted of driving a motor vehicle on a road after consuming so much alcohol that the proportion of it in his breath exceeded the prescribed limit. Furthermore, that Dr Banks was sentenced to a community order with a duration of 6 months.

Honnir, ar 17 Rhagfyr 2018, yn Llys Ynadon Caernarfon, bod Dr Banks wedi’i gael yn euog o yrru cerbyd modur ar ffordd ar ôl yfed cymaint o alcohol nes bod y gyfran ohono yn ei anadl yn uwch na'r terfyn penodedig. Ymhellach, y dedfrydwyd Dr Banks i orchymyn cymunedol â hyd o 6 mis.
