Pre hearing information

Allegation

The tribunal found that, on one or more occasion on 30 March 2018, Dr Alam retrospectively inserted an entry into one or more patient’s medical records to state that the patient had been given a chronic disease monitoring verbal invite when he had not issued such an invite and had not seen and/or reviewed the patient. The tribunal found that his actions were misleading.

The tribunal also found that on one or more occasion between 30 and 31 March 2018, Dr Alam exception reported patients from Quality and Outcomes Framework indicators having altered their medical records, and that his actions in doing so were misleading.

The tribunal will reconvene to determine whether Dr Alam’s fitness to practise is currently impaired by reason of misconduct.
