Allegation

The tribunal will inquire into the allegation that, whilst employed in the Gastroenterology department at Nevill Hall Hospital between February 2017 and May 2017, Dr Zafar failed to work collaboratively and communicate appropriately with colleagues, failed to carry out urgent actions and respond to bleeps, was at times unavailable for work without prior approval or adequate reason and failed to demonstrate insight into concerns raised. It is further alleged that Dr Zafar acted dishonestly by claiming that his educational supervisor had made comments regarding his Multi Consultant Report when his supervisor had not made those comments.

It is also alleged that, in February 2017, as part of four separate applications for Higher Specialty Training posts, Dr Zafar dishonestly submitted self-assessment forms in which he made untrue statements, and that he later dishonestly made an untrue statement about these forms at a meeting with Wales Deanery. It is further alleged that, in May 2017, Dr Zafar sent emails that were inappropriate, unprofessional and demonstrated a lack of insight on three occasions. It is also alleged that, on or around 14 May 2017, Dr Zafar applied for a position at Aintree University Hospital Trust where he failed to disclose that he had been excluded from clinical activities at Nevill Hall Hospital, had not completed his Core Medical Training and that his Annual Review of Competency Progression had been deferred. It is further alleged that, during a meeting on 24 August 2017, Dr Zafar dishonestly made untrue comments about providing information about being under investigation and being suspended from clinical duties at Nevill Hall Hospital prior to commencing employment at Aintree University Hospital Trust.
