Pre hearing information
The tribunal will inquire into the allegation that, between February and May 2017, whilst employed in the Gastroenterology department at Nevill Hall Hospital, Dr Zafar failed to work collaboratively with colleagues.
It is further alleged that in February 2017, in applications for Higher Speciality Training posts, Dr Zafar submitted self-assessment forms in which he made statements that were untrue. It is alleged that Dr Zafar’s actions were dishonest.
Finally, it is alleged that in May 2017, Dr Zafar applied for a position at Aintree University Hospital Trust and between May and August 2017, after taking up the position, failed to disclose information relevant to his employment. It is alleged that in a meeting on 24 August 2017, Dr Zafar dishonestly claimed to have informed the Trust of the relevant information prior to commencing his employment.

Yn olaf, honnir ym mis Mai 2017, bod Dr Zafar wedi gwneud cais am swydd yn Ymddiriedolaeth Ysbyty Prifysgol Aintree a rhwng mis Mai a mis Awst 2017, ar ôl iddo dderbyn y swydd, methodd â datgelu gwybodaeth sy’n berthnasol i’w gyflogaeth. Honnir mewn cyfarfod ar 24 Awst 2017 fod Dr Zafar wedi honni’n anonest iddo roi gwybod i’r Ymddiriedolaeth am y wybodaeth berthnasol cyn dechrau ei gyflogaeth.
This reflects the allegation as it stands at the start of the hearing. The allegation may be amended as the hearing proceeds and when findings of fact are made by the tribunal.
