Pre hearing information

Allegation

The tribunal will inquire into the allegation that on 26 October 2018, whilst working in the renal department of Hammersmith Hospital, Dr Das issued inappropriate prescriptions to Patient A. It is alleged that Dr Das: demonstrated poor clinical knowledge in her prescribing; failed to adequately discuss Patient A’s prescriptions with senior colleagues and failed to keep adequate records in respect of Patient A.

It is further alleged that from the commencement of her employment with Imperial College Healthcare NHS Trust (ICHT) in August 2018, Dr Das failed to follow proper procedure to obtain an NHS Smart Card for access to patient records. It is also alleged that Dr Das inappropriately used the name of Dr B when ordering prescriptions, conducting investigations and completing discharge summaries and that her actions in this regard were dishonest.

Finally, it is alleged that during her employment at ICHT and London North West Healthcare NHS Trust, Dr Das posted comments on social media on multiple occasions which were inappropriate and in breach of her employers’ social media policy.
