Pre hearing information

Allegation

The tribunal will inquire into the allegation that, on 11 February 2020, Dr Veleshala failed to provide adequate care and treatment to Patient A. It is alleged that Dr Veleshala’s failings related to history taking, examination, assessment, investigations, diagnosis, review and record keeping.

It is further alleged that Dr Veleshala subsequently amended her notes of Patient A’s treatment without indicating that the amendments had been made retrospectively and that in 2021 Dr Veleshala gave false evidence to the Coroner. It is alleged that Dr Veleshala’s conduct was dishonest.
