Pre hearing information

Allegation

The tribunal will continue to inquire into the allegation that on 15 October 2019 Dr Haider completed two blood Transfusion Request forms (the Forms) in relation to Patient A. It is alleged that Dr Haider acted dishonestly when completing the Forms.

It is further alleged that on or around 2 December 2019 Dr Haider asked Dr C, who was not her clinical supervisor, to sign off her work-based competency assessments. It is alleged that Dr Haider’s actions were dishonest as she was aware that work-based competency assessments should be signed off by a direct clinical supervisor.
