- Allegation
The tribunal will inquire into the allegation that, between August 2019 and March 2024, Dr Mutlak completed Procedure Based Assessment forms (PBAs) in which he had included false information and forged the signatures of other individuals.
It is further alleged that, on 7 March 2024, Dr Mutlak submitted the PBAs to the GMC to support a Certificate of Eligibility for Specialist Registration application. It is alleged that Dr Mutlak’s actions were dishonest.
