Pre hearing information

Allegation

The tribunal will inquire into the allegation that, on more than one occasion between February 2021 and October 2022, Dr Collin prescribed medication for himself.

It is further alleged that, during the period May 2015 to September 2023, Dr Collin issued prescriptions, including controlled drugs, in a non-emergency situation to four patients with whom he had a close personal relationship. It is alleged that Dr Collin subsequently attempted to conceal the prescriptions by cancelling them after they had been printed, giving the false impression that they had not been issued. It is alleged that Dr Collin’s actions were dishonest.

Bydd y tribiwnlys yn ymchwilio i’r honiad bod Dr Collin wedi rhagnodi meddyginiaeth iddo’i hun ar fwy nag un achlysur, rhwng mis Chwefror 2021 a mis Hydref 2022.

Ceir honiadau pellach fod Dr Collin wedi rhoi presgripsiynau, gan gynnwys cyffuriau rheoledig, mewn sefyllfa nad oedd yn argyfwng i bedwar claf yr oedd ganddo berthynas bersonol agos â nhw, yn ystod y cyfnod rhwng mis Mai 2015 a mis Medi 2023. Honnir bod Dr Collin wedi ceisio cuddio’r presgripsiynau drwy eu canslo ar ôl iddynt gael eu hargraffu, gan roi’r argraff nad oeddent wedi cael eu rhoi. Honnir bod gweithredoedd Dr Collin yn anonest.
